45 year old female was admitted in ED with pain in abdomen and vomiting of two days duration Amylase and lipase is elevated. USG shows multiple gall stones. LFT- Bil 4.5/3.5, OT-110, PT-120. TLC count is 12000. Next management A- IV fluids, Octreotide, NPO B-IV fluids, tramadol C-IV fluids, IV antibiotics and ERCP with stent placement D-MRCP

Explanation:

Diagnosis is Biliary pancreatitis = mild

MANAGEMENT OF MILD / MODERATE PANCREATITIS:

  • IV Antibiotics no role without sepsis
  • ERCP + stent = if only CHOLANGITIS IS present
  • OCTREOTIDE = No role
